Waiver Case Management
One of MNCCS’ successful partnerships has been a contract with Ramsey County to provide culturally specific waiver case management services for residents from Somali and other east African immigrant communities. Under this contract, MNCCS provides case management for persons with disabilities who receive funding through the CADI, B.I., CAC, or D.D. waiver (or Rule 185 targeted case management). Our providers are fluent in Somali, Arabic, Oromo, Amharic, and/or Swahili, as well as English, and have a unique ability to provide waiver case management services in participants’ native languages. Our staff also participate in cultural responsiveness training that helps them work across differences and serve the general population. We are the provider of choice for Somali participants but have also worked very successfully with people from multiple races and backgrounds in our Ramsey County work.
