What We Do
245D: Home Community Based Service
Waiver services to help people maintain community independence. MNCCS is a 245D HCBS provider and has a contract with Ramsey County to provide waiver case management for over 65 people. We operate a child foster care respite home in Bloomington that provides up to three days of overnight respite care for children with autism. It serves both the east African immigrant community and the general public.

Providing services to people experiencing homelessness. MNCCS began providing Housing Supports in 2016 and today has Housing Support contracts for 150 scattered site units in four counties, as well as the ability to provide Housing Stabilization Services through insurance. Our agency is experienced with all aspects of Housing Support, including outreach, transition, and tenancy sustainment.

One of MNCCS’ successful partnerships has been a contract with Ramsey County to provide culturally specific waiver case management services for residents from Somali and other east African immigrant communities. We are the provider of choice for Somali participants but have also worked very successfully with people from multiple races and backgrounds in our Ramsey County work.

Mental health services to help people with serious mental illness improve their ability to function in the community. MNCCS provides ARMHS in the metro area as well as in Faribault, Owatonna, and Rochester, currently serving 170 participants. Our ARMHS practitioners work in home and community settings to provide rehabilitative services. Many of MNCCS’ housing clients live with serious mental illness and qualify for ARMHS. Our mental health professionals provide supervision and can assist with D.A.s. MNCCS also offers trauma-informed, culturally responsive psychotherapy and medication management.

Please take a moment to peek at the highlights of the Newton out-of-home Respite Program in Bloomington. Our program supports up to 4 youths at one time, up to age 18. The kids that visit enjoy a comfortable atmosphere, therapeutic recreation and care, supervision, well-balanced meals made from mostly non-processed foods, and social dialogue. Our Newton Respite Program offers individuals and their families a unique respite option that gives participants a relaxing, safe, and fun respite environment while allowing families or care providers a break from their daily demands.
